OE's Renewed Horizons Adult Day Center provides a planned program that includes a variety of health, social, and support services in a protective daytime setting.

This is a community based service designed to meet individual needs of functionally impaired adults through individualized plans of care. Individuals may receive personal care and assistance, medical consultations by an agency nurse, and social/recreational activities.

Eligibility Requirements:

Resident of Porter County or surrounding county;
At least 50 years of age;
Diagnosed with Alzheimer's or other physical/mental disorders;
Able to be medically managed;
Free of communicable diseases;
Able to provide appropriate funding.

Transportation is also available based on the location of pick-up.

Renewed Horizons is a member of the Indiana Association on Adult Day Centers and the Indiana Association of Adult Day Services. The center is also certified by CARF, the Commission of Accreditation of Rehabilitation Facilities.
